Florida Medical Tourism

Florida Medical Tourism is a unique global service in medical care. It was originally designed to offer special medical services to global customers, specifically from the Middle East (Arabic speaking areas). The program was founded in the spring of 2012 to offer culturally sensitive medical services with emphasis on universal values and compassionate care.

A very significant global event will take place, most likely on June 17, and extend over a month. That is the arrival of the holy month of Ramadan.

During this month, there is spiritual renewal, redemption of all good deeds and endless mercy from our creator — the great prize of which will be the ultimate freedom from hellfire.

From the minute it starts, more than 1.7 billion followers of Islam engage in continuous worship — fasting during the day, praying till midnight and charitable giving.

This month reflects on thanking God Almighty for his endless bounties by sharing with the less fortunate. At sunset every day, the breaking of the fast, called Iftar, takes place across the world. Food is shared, and people gather to thank God for providing it.

As a matter of fact, for the most part, anyone can walk over to a mosque or Islamic center and eat for free. This is the spirit of Ramadan.

Fasting teaches us patience, discipline, self-control and humility, and its gets rid of arrogance and impatience. It does not distinguish between the richest man or woman and the poorest. Everyone is to fast and feel hunger and thirst during the period from sunrise to sunset for a month to feel the pain of others and appreciate the blessings bestowed upon us.

With God Almighty's acceptance of all good deeds — including, but not limited to, praying, charity, helping the needy, feeding the hungry and serving the community — all of our sins are erased, and we become pure again.

There are many health benefits to fasting. It helps control chronic diseases and also helps with weight loss, thus cutting down the risk for cancer and many other diseases. Fasting gives the body its annual maintenance to shed dead cells. In fact, the Department of Health in England has incorporated fasting as one of its wellness strategies.

Additionally, fasting helps get rid of stress, depression and anxiety, and it brings peace, tranquility and happiness. So everyone is encouraged to practice intermittent fasting for health and wellness, and it does not have to be as strict as the Muslim practice.

Ramadan softens the heart to make reconciliations with one's self, family, community and the world, and to be a good servant of the creator, helping fellow man unconditionally. The celebration peaks during the last 10 days, as everyone looks forward the Night of Power on the odd nights, where the angel Gabriel and others descend close to Earth.

Then, you come to realize that this fast-passing month will end soon, and we say goodbye until the next Ramadan comes.

Then the feast of breaking the fast, also known as Eid al-Fitr, comes as a trophy, along with the celebration for doing a good job during Ramadan. It includes buying new cloths, parties, rides for kids and exchanging gifts with family members and friends.

Ramadan is a blessed month for all mankind, with so many physical, mental, and spiritual benefits. It is also a great opportunity to abstain from bad habits such as smoking, overeating, lying, cheating, backbiting and envy, offering a chance to transform yourself to be a better human being.

Everyone can benefit from Ramadan and make this month a special free gift to all humanity across the world.

Transforming Healthcare!

Watching the debate on optimal healthcare delivery with various parties including CEOs of small hospitals in rural America threatened by closure due to financial difficulties and an inability to keep up with rising costs and decreased reimbursements . Unfortunately they happen to be serving the most disadvantaged Americans who are  challenged with poverty and lack of education and have high rate of chronic diseases and complicated medical issues.Now they are having more difficult access to healthcare. The notion that Tele-medicine and monitoring devices given to patients will replace the traditional care in these particular rural areas may not result in achieving the target goals unless it is also combined with human elements that will make sense of all the data that is being collected from the devices and make a meaningful impact on their care. By properly educating them how to use these gadgets. 

There is no question that technology can play a major role and can fill in a part of the gap that has been created by the huge shift of healthcare focus to implement prevention and wellness rather than acute care management or (spot fix) with the shortage of physicians.

On the contrary there is a surge of retail medicine clinics  noted on the rise like mini clinics at CVS , Wal-Mart and others that offer convenient access to the patients and replace the traditional care visits such as ER visits.

This trend  is expected to continues to increase all across America , and even helped by the Affordable Care Act( Obama Care).

Now, It is up to the consumer to choose what is the easiest, most convenient hours and locations and the best prices and be  engaged in their healthcare decisions and be educated and have true partnership with an ongoing discussion that will keep them healthy and save them money and prevent expensive ER trips and hospitalization.

My Very First Patient!

I vividly remember when I was so excited to have finished my very long tough 10 years of post graduate training going through clinical Internship, Residency , Research Fellowship then Clinical / Interventional Cardiology Fellowship in Long Island, New York City as I was moving to Florida to finally start working in Private Practice and everyone was envying me for going there!

I could not understand why as the first three summer months went like this , it was almost raining everyday, hot , humid , fogging of my glasses and moving in and out wet even with the umbrella to air conditioned areas and getting cold and my oldest daughter who was got sick with allergies , almost stopped breathing and had to be put in an oxygen chamber, but thank God , she got better and has been ok since then the weather started to cool off and sky turn beautiful clear blue and then saw the snowbirds coming in  to Florida from all over the United states and Canada and the rest of the world,so I started to appreciate being in Florida after a rough start!.

Then , I remember  my very first patient who was introduced by his wife and both became like family since!. We used to laugh a lot when his chart was the only one in the rack ( in the old days before Electronic Medical Records !) as he was jokingly suggesting to spray the rack with (Miracle Grow) because for few months , his chart was the only one in the chart rack!.  Well,he noted on subsequent office visits that that the rack was getting  packed with charts and racks had to be upgraded many times to accommodate the ever increasing number of patients coming to my humble Cardiology/Internal Medicine practice.

So, through 17 years of our relationship with him and his wife, he became the star of all our practice events such as annual Health Fairs, Business Mixers with the Chamber of Commerce and other community events where he proudly share his positive experience with our practice.  He spoke not only about the medical services he received as he was treated successfully for critical blockages in one of his main coronary arteries and I got that fixed with putting coronary stents and he did great since then with no further complains of chest pain or need for any further procedures on aggressive medical regimen. But also, he spoke also about how he was treated with compassion and saved  so much unnecessary healthcare spending by coming to our practice . He spoke about how he enjoyed the visits , and usually after covering the medical issues, he would share his life , job, kids and grand kids, plans, dreams and he never forgot about his ( Miracle Grow joke)!

I speak multiple languages among which is Arabic . The physician can be called either doctor or (Hakeem) which means ( the Wise man or Hakeema )for a lady doctor) reflecting added quality built in the doctor in addition to medical skills is to be able in general to make and give people  wise decisions!.

So patients may seek advice on personal life, marriage, kids, finances or simply find a shoulder to cry on, and I joke with my patients who carry on these discussions off the medical issues that I am not a religious authority or a marriage counselor or a social worker and we all have a good laugh!

My patients call me not infrequently asking me to pray for them when they go through tough times or loosing loved ones and I never turn anyone down making them at ease and provide them with comfort when needed most.

This human interaction for me is the real joy and honor of practicing this noble profession in addition to providing superb professional services. Being a doctor was never about money and lavish life style. It is about serving and helping people and if your patients see and feel that , they will follow you to the moon!

Sadly, with all the non-sense that is happening in Medicine, I strongly believe in the direct care Medicine between the patient and his doctor with no hassle or control of the third party is the way to go. Just the old fashion way with this strong relationship where the patient trusts his/her doctor and the doctor educate, advocate and serve his patients with the highest ethical standards.

To be able to actually concentrate on doing  what we were trained to do which is to deliver the best care of patients , save them money and  treat them like family.

Happy Mother's Day

We all know that everyone in the whole universe came to life from their mother's womb with the exception of Adam and Jesus (peace be upon them). Thus, there is a very special status for mothers out there as they are truly the glue that keeps the family together and give love, attention, and care for her children. 

If you are sick, hungry, or feeling stressed or just simply want to talk about what happened at school or at your job you rush to your mom. If you can't go to sleep or had a bad dream that woke you up, you will call for your mother to come and comfort you and so on. 

We will never do justice to our mothers by celebrating her on just one day or just take her out to dinner once a year, but, everyday should be filled with honoring mothers for their endless contributions whether they are living with us or if they have passed on. To appreciate their non-stop giving of unconditional love to their children. 

My Mother had a great impact on my upbringing and in choosing medicine as a profession to help mankind. She stayed up with me late at night to study for my exams and prayed for my success and happiness. I owe her a lot as all what I have accomplished was due to her prayers that God has answered. 

Now I see a new focused marketing campaign to target women and mothers as they are considered the main decision-makers for health care choices for the family. And thus, they will be reached out to by major insurance companies, hospitals and providers to secure their market share. It would be great to give all hard working mothers a break, especially in the middle class situation as they struggle to make ends meet. This is usually the case for single-mom families to make healthcare really affordable would really help them out. 

The mothers put everybody in the families needs ahead of her own needs including healthcare needs. She may not even have any coverage as she would be faced with tough choices whether to put food on the table or to provide healthcare coverage. And if she collapses the family collapses, and thus this is our society.

Preauthorization Medicine!

The full and absolute control of third party medicine in anything and everything has to do with the care of patients is real . Now this concept is going through a gateway of authorization. Preauthorization for any medical test ,procedure, office visit, medication to be dispensed, hospitalization, need of  Medical devices, durable equipment, oxygen, cane, walker, shoes etc. and the list goes on. 

The daily pains of so many physicians and their staff as they go through this everyday in an attempt to serve their patients; getting them what they need and well indicated and providing them with the appropriate medical necessities. 

When being confronted with preauthorization doctrines imposed by insurance companies for which the first response in about 99% of cases would be a denial. If a physician is to fight for their patient he or she may get lucky to get authorization for a test or a procedure that their patients  need on the third time around. Make no mistake about it you will be given an initial optimistic response "Oh yeah, its all covered by insurance" to find out the painful reality of going through the insurance system of preauthorization. For example, 88 year old medicare patient who has very high blood pressure and was put on so many blood pressure medications that failed and ended up in costly hospitalization that could have been avoided if the patient was given the specific medication that was appropriate to her condition.  After 18 faxes, 7 phone calls with the pharmacy explaining that this was the only medicine the patient could tolerate the answer was still that we had to call the insurance company to get it approved from which we would be given a case number. After calling the 1-800 number and  explaining for 25 minutes why the specific medication should be authorized for the patient we were told that this request would be reviewed by the pharmacy committee and the decision would appear in 24 hours. After the 24 hours however, the request for authorization was denied. As for this frail elderly patient was told that they would have to pay $6 a pill which they could Not afford as it was out of pocket although, this patient should have been given the medication with 100% coverage between medicare and her insurance. 

The time and energy wasted, the pain and suffering of this senior patient waiting for their medication to be authorized knowing that if patient doesn't receive it that they will be hospitalized, causing the system  thousands of dollars which could have been avoided if the medication had been authorized as it was made available in low cost generic form. 

Similar stories like this happen to patients across the country on daily basis  proving how inefficient, costly, and stressful this system to both patients and caring providers. The saga goes on for this elderly cardiac patient at no avail. 

Preauthorization medicine was designed to save money to insurance companies on high utilization providers but patients are not getting the care they need or deserve. Someone will never be given the preauthorization on therapy or a procedure that could be lifesaving which will be delayed and pushed back and in the mean time the patients condition may deteriorate and in some cases may never get the therapy or life saving procedure until they are deceased by the time they get the authorization!. 

The evidence -based  Medicine clinical guidelines are already well established for each disease category and therefore , they should be the guiding force for preauthorization medicine.

Happy Doctors Day!

When I met one of my colleagues as he was greeted with ( Happy Doctors Day), he replied, what is so happy about it? He went on and explained that he saw recently about 50% of private practice physicians gone!. They sold out their practices since they could not survive the rising operational cost to run their small businesses with ever decreasing revenues to the point that they had to sell out their practices mostly for Hugh losses. He added there is more than 65% of physicians are suffering from ( burnout syndrome) as they work so much harder for less revenue , more regulations, mountains of bureaucratic paper work that is using up their precious time that should be focused on one thing, which is taking care of patients!

Physicians see themselves loosing autonomy to make appropriate decisions for the care of their patients as so many of them became employed and they have to do what they were told to do if they want to keep their jobs. As they go through daily uncertainty if they were going to keep their jobs or going to be replaced with lower pay personnel ( physician extensors) as cost -saving measure by the employers!

Even thousands of the employed physicians were terminated instantly and a lot of them did not even know about it and why and you can imagine the devastation to those physicians and their families.

According to recent surveys, more than 50%of physicians will not recommend their kids to become physicians, as the very demanding life style being on call, long hours of work , so many years of medical school and training and of course the big student loans to name a few. The myth about doctors making big money has been gone when CMS , the Center for Medicare Services published online the different pay scale categories , showing the top earners were the CEOs and Administrators , and the physicians came in the least earning category, although they control 100% of the healthcare expenditure. Additionally, physicians have lost much of their ability to express their concerns in fear of being labelled as ( disruptive physicians and risk to loose license to practice medicine)!

You may add, he explained the rising of Retail Medicine by big companies which will end up hiring mostly Nurse Practitioners rather than physicians for bottom line profitability. You can forget about innovations and Entrepreneurship which made America lead in the world as many physician practices continue to close and physicians are pushed to either earlier retirement or simply give up and get consumed only about their livelihood !

All this is taking place at a time when we are suffering from big shortage of thousands of physicians with more baby boomers needing healthcare services, although some proposed Tele Medicine to be the answer to physician shortage which is not realistic. To make things worse for the remaining practicing physicians especially in the small and medium private practices have to get ready for the ICD9 to ICD10 conversion anticipated to take place in October this year, only few months away with some earlier reports predicting that claims denial rate by CMS could reach 100% with Hugh IT issues as the control of tremendous amount of health data by various IT vendors with only big vendors dominate and the small IT vendors helping small physician practices will crash, HIPPA issues and others , which will have additional cost burden estimated to be tens of thousands of dollars coming at a time of anticipated interruption of payments , see who is going to survive that double whammy!

After listening attentively, I started asking my colleague to first take three deep breaths with me slowly in and out to help relax and then I wanted to share with him the bright side of choosing Medicine as a Noble profession with acknowledging all the current challenges facing physicians, especially those in small private practice who traditionally delivered most of the healthcare services to patients over the years .

I told him that there is always going to need for Physicians , no matter what anyone says!, and that patients are always looking for that caring and compassionate doctor who delivers healthcare efficiently. Also patient value those qualities in the doctor caring for them and they do not mind paying for these services . Additionally, there is always going to a need for the human component of healthcare , no matter how fancy the digital medicine and technology apps get. There is always a golden rule if you do what is right and ethical , you are going to do well as a physician .

I added , if you or any other colleagues are looking for a dignified model for the practice of Medicine, then it will be the Direct Care Model or Third Party- Free Medicine where is a trust by the patient that their doctor will do his or her best for the patient and doctors honors and respects his patient to deliver the most valuable service( health). I always encourage young students to go to Medical School if they have the passion and calling for this great profession to be able to serve and help people. If you do that then you will enjoy both on professional and economic aspects , you may not make tons of money , but you will have enough to live well.

Preventative care

Chronic diseases are the leading causes of death and disability in the United States, advises the Centers for Disease Control and Prevention (CDC).

Chronic diseases are heart disease, stroke, cancer, diabetes, and arthritis and are the most common and costly of all health problems for Americans.

These chronic diseases are also the most preventable, stressing the importance for early detection.

Some believe early detection, like an ounce of prevention, is worth a pound of cure.

One Hernando county physician has implemented a practical approach for local patients to spot small problems before they become big ones.

Dr. Adel M. Eldin, M.D., specializing in internal medicine and cardiology, his practice is Brooksville Cardiology, explained his health and wellness program, which focuses on preventative care. He presented his program to more than 50 participants gathered at Palace Grand in Spring Hill, listening to testimonials from patients who have experienced success with the cardiology center.

"It's possible to detect problems before there's an emergency and 95 percent preventable, given our knowledge, medications and lifestyle modifications," Eldin said.

But there's one problem, Medicare and other insurance companies don't cover preventive screenings without the presence of symptoms, he advised.

"Insurance focuses on illness, rather than wellness," he said. "Wellness is there for you."

"When you invest in your health, you won't have to be part of the insurance jungle out there." Eldin said. "I've spent five years introducing a new program that identifies risks, teaches preventive care and provides an on going partnership in healthy living."

Brooksville Cardiology is a medically supervised program to prevent disease and guide people into health-related choices. They review family and medical history, assess current status, identify areas that need attention and implement a plan. The patient is involved in the decisions for well being. His staff is trained to perform screenings, non-invasively to look inside arteries for plaque buildup that can cause strokes (brain shock), aneursyms and heart attacks.

"Dr. Eldin showed me more compassion than any doctor my whole life," said Errol Wiles. "I signed up for this program two months ago. It's been a positive experience."

Covered medical services include two office visits for routine check-ups, baseline cardio-vascular testing including treadmill exercise stress test, echocardiogram, vascular studies, 24-hour Holter monitor testing, monthly Coumadin check, interrogation of cardiac devices, annual flu shot, year round supply of recommended supplement of Black seed, e-newsletter, personal access to physician through email up to one time per week and referrals to other specialists when needed. Members also receive access to a webinar which features educational programs on health.

There are no denials for coverage under the plan for pre-existing conditions and no additional payments are needed for tests such as co-pays. Additionally, there are no referrals or pre-authorizations for testing. Open enrollment is available throughout the year.

"This is the way health-care is headed," he said. "We want to boost the immune system and prevent diseases."

One supplement he recommends is Black seed, or Nigella sativa. It's also referred to as black cumin.

"Nigella sativa (black seed) is an important medicinal herb. In many Arabian, Asian and African countries, black seed oil is used as a natural remedy for a wide range of diseases, including various allergies," according to a study found on the U.S. National Library of Medicine and National Institutes for Health website.

Black seed oil proved to be an effective adjuvant for the treatment of allergic diseases, based on the research.

The seeds/oil has anti-inflammatory, analgesic, antipyretic, antimicrobial and antineoplastic activity. The oil decreases blood pressure and increases respiration, according to other research regarding its pharmacological and toxicological properties.

The seeds have been used in the Middle East to treat asthma, bronchitis, rheumatism and related inflammatory diseases.

Researchers at Kimmel Cancer Center, Thomas Jefferson University in Philadelphia have studied that thymoquinone, an extract of N. sativa seed oil, blocked pancreatic cancer cell growth and killed the cells, acting as a Histone deacetylase inhibitors (HDAC) for use in psychiatry and neurology as mood stabilizers and anti-epileptics

Studies are in the early stages.

"Black seed has been around for thousands of years," said Dr. Eldin. "It is an old remedy mentioned in the Bible and was found in King Tut's tomb."

Black seed is not recommended for patients undergoing treatments for a medical condition or if they are pregnant or lactating, according to information provided by Envirotech International, Inc., the manufacturer. More information can be found at

So when patients do the math, one Visit to Emergency Room average cost about 5 Thousand dollars which will be equivalent to 10 years enrollment for someone between 20 to 30 years of age! and one day Hospitalization average cost 15 to 20 Thousand dollars which will equal to 40 years , yes 40 years of enrollment in my plan for the same age group , and you can do the math for the rest of the various age groups , so when the patients are educated , the decision which they will make during this tough economy will be so easy. This goes to support that Concierge Medicine program that I developed was really developed to help the middle class hard working Americans , and dispel the myth the Concierge Medicine is for the rich! ( not true ) and when you put a few dollars a day less than $ 5 /day to invest in your health would make a great investment compared with what you have to spend elsewhere with emphasis on Disease prevention, Comprehensive Wellness and Stress management.

As Healthcare is going to continue to be driven by consumerism with current impact of social media that makes patients (consumers) more educated about choices in healthcare including Concierge Medicine. A good example of that is the positive impact of Affordable Care Act on the growth of Concierge Medicine as many big corporates such Wall mart, CVS, and soon Target will also provide Concierge Medicine in Retail Clinics as needs grows!

Concierge Medicine allows doctors to concentrate on the practice of medicine and provide a personalized quality services with their patients as they are NOT pressured compared with traditional practice ( usually half number of patients seen per day) , so it is good for the patient and the doctor as well who makes a fair compensation for years of education and training!

In brief , I think there will be further growth in Concierge Medicine in 2015 , It makes sense and that is why some Health Insurance carriers like Cigna made Concierge Medicine available for employees and made it complaint with ACA as it made Direct primary care ( Concierge Medicine) bundled with catastrophic coverage Policy . The holy month of Ramadan begins Saturday, (June 28) and is a great opportunity for spiritual renewal with physical and health benefits.

Muslims fast from sunrise to sunset with no food or drink and have nightly prayers called Taraweeheh, (joy of the heart prayer). Department of Health guidelines in England included fasting as a way to promote comprehensive health to combat chronic diseases as diabetes, cholesterol, heart disease and degenerative arthritis.

Among the benefits of Ramadan fasting:

  • Purification of the heart because eating so much causes almost a blinding of the heart. It makes thinking and concentration difficult and makes the body sluggish and the ability to extract, memorize and analyze data is much slower. Purity of the heart and mind, achieved by fasting, helps sharpen up the brain and memory as well as the heart.
  • Softening the heart because there is frequent prayer and remembrance of God that improves mental and psychological status.
  • Removing arrogance from a person's behavior. While fasting, a person realizes how the human being is helpless and should appreciate all the bounties given from our creator. You realize that there is no room to be arrogant or feel like you are above everybody else.
  • Not forgetting the testing of God. You will know that the person who eats a lot forgets about the hungry and forgets about the thirsty. When you go through the hunger pain and the thirst, for limited hours, you will be able to relate to so many unfortunate fellow human beings who don't have all luxuries we have.
  • Combatting desire and temptation. When you eat less, you will have less temptation and you will be able to control your desires. Hunger will limit sexual desire as well as help abstinence of the tongue which includes backbiting, lying and cheating others. It also will help stop constant chocolate cravings.
  • Ridding yourself of activities and distractions that revolve around the eating such as grocery shopping and cooking. You will have more time to spend with God.
  • Limiting food-related illnesses like indigestion, distention, gas and reflux disease. Fasting also helps regulate sleeping patterns because the less one eats the more energized one feels.
  • Saving money because when you eat less you will not spend money beyond your budget. You will adapt to only eating small amounts of food, and avoid extravagant spending.

Ramadan is about sharing what God has given you with the orphan, the poor and needy because that charity will help you in this life and hereafter. Ramadan brings inner peace and happiness due to the release of natural endorphins produced with fasting, praying in large amounts and doing good deeds. Fasting is a true gift from God, and should be practiced and shared by everyone.

You don't have to do this strict fasting if you are not an observing Muslim in the month of Ramadan but you could have liquid-only diets (smoothies or shakes) rather than solids, leading to weight loss to combat obesity and fight stress. It is a true effective therapy for the human being. And, please share a sunset meal with your Muslim neighbors at any Mosque because it is the month of mercy, compassion and charity for entire humanity.

Peace on Earth for all of us

I recently had a phone call from Dan DeWitt asking about Muslims taking part in the religious display at the courthouse in Brooksville and I mentioned to him that this Christmas holiday is the opportunity for our fellow friends of Christian faith to express their religious holiday and rightfully so enjoy it and I mentioned to him that the love of Jesus shared by Muslims and Christians alike should bring them closer than ever this holiday season.

I thought it would be beneficial to share some facts that Muslims love and respect Jesus as prophet of God who was sent with guidance to the people of Israel and who is mentioned in the holy Koran more times than the prophet of Islam, Mohammed (Peace be upon Him).

It is very interesting to notice that Christians know the holiest places in Christianity in Jerusalem, but very few know that it is actually two Muslims who open and close the holy door to these Christian sites and serve as the custodians of the entrance. Their names are Adeeb Joudeh and Wajeeh Nuseibeh, both are Palestinians and Muslims bearing the key to the heart of Christianity.

Watching world events and the amount of human suffering and all conflicts created out of ignorance and manipulated under religious context, one comes to realize that the only viable path to achieving peace is by education and dialogue, not by wars and killing.

Although Muslims do not celebrate Christmas, we do celebrate the message of peace of all prophets on a 24/7 basis. Let us work for peace in the season to make peace on Earth a reality not just in the Christmas card.
